Colorado

Engagement: Cohort 2015

Challenge: How to share data among eligible families, providers, and state agencies receiving care coordination services.

Tools: Whole Systems Mapping, Process Flow Diagrams, Impact Matrix

Project Results: Colorado made it a policy requirement that all Title V-funded agencies create a care coordination plan that they share the plan with the family and at least one other agency involved in the family’s care. A care coordination plan is also entered into the state’s data system.

Outcome: With the data sharing system in place, the Title V agency observed a 45 percent increase in agencies that were doing systems level and policy work around care coordination.

Engagement: Single-State Intensive 2016

Challenge: Eliminate system-wide behaviors that discourage CYSCHN families from receiving services

Tools: Hexagon Tool, Stage Analysis, PDSA Cycles

Project Results: A pilot-project was designed to increase the efficiency and avoid duplication of care among CYSCHN populations receiving services through Title V and Healthy Communities, and Medicaid’s Accountable Care Collaborative Program.

Outcome: Created a template for systemic changes aimed at addressing the social determinants of health.

Engagement: Cohort 2019

Challenge: Re-orient the state’s Title V efforts to address social determinants of health

Tools: 30 30s, Aim and Challenge Statements, Network Mapping, System Support Maps

Project Results: The work will address the scope of maternal wellness work and the skillsets and training of the state’s Title V staff and form the basis of the state’s maternal and child health priorities for the 2021-2025 Title V Block Grant process.

Outcome: In progress.
